Envelope oligomerization is thought to serve crucial functions during the life cycle of human immunodeficiency virus type 1 (HIV-1). We recently reported that virus entry requires coiled-coil formation of the leucine zipper-like domain of the HIV-1 transmembrane envelope glycoprotein gp41. To determine the oligomeric state mediated by this region of the envelope, we have expressed the zipper motif as a fusion partner with the monomeric maltose-binding protein of Escherichia coli. The biophys. properties of this protein were characterized by velocity and equilibrium sedimentation, size exclusion chromatog., light scattering, and chem. crosslinking analyses. Results indicate that the leucine zipper sequence from HIV-1 is capable of multimerizing much larger and otherwise monomeric proteins into extremely stable tetramers. Recombinant proteins containing an alanine or a serine substitution at a critical isoleucine residue within the zipper region were also generated and similarly analyzed. The alanine- and serine-substituted proteins behaved as tetrameric and monomeric species, resp., consistent with the influence of these same substitutions on the helical coiled-coil structure of synthetic peptide models. On the basis of these findings, we propose that the fusogenic gp41 structure involves tetramerization of the leucine zipper domain which is situated ∼30 residues from the N-terminal fusion peptide sequence.

The effect of carboprost tromethamine (hemabate) combined with modified patch suture in placenta previa postpartum hemorrhage was observed Sixty-two patients with placenta previa were divided into the control group (30 cases) and treatment group (32 cases). The control group was treated with hemabate, and the treatment group was treated with hemabate combined with modified patch suture. The two groups of intraoperative, postpartum 2 h, 24 h postpartum were compared in bleeding volume and Hb changes. The intraoperative bleeding volume [(321.9±67.8) ml vs (435.9±83.3) ml] and 2h, 24h postpartum bleeding volume [(417.7±68.0) ml vs (539.6±70.4) ml and (485.3±68.5) ml vs (621.0±81.3) ml] of the treatment group were significant less than that of the control group (t=5.926, 6.938, 7.133, all P<0.05). Significant difference in changes of Hb level was also observed between the two groups, though postpartum level was not significantly reduced in the treatment group compared with the control group [(5.0±6.5) g/L vs (-4.0±6.8) g/L] (t=-3.662, P<0.05). Hemabate combined with modified patch suture can be more effective in the prevention of postpartum hemorrhage in placenta previa.

A subset of eukaryotic aminoacyl-tRNA synthetases (a-RS) are contained in a multienzyme complex for which little structural detail is known. Three reversible chem. crosslinking reagents have been used to investigate the arrangement of polypeptides within this particle as isolated from rabbit reticulocytes. Identification of the crosslinked protein pairs was accomplished by two-dimensional SDS diagonal gel electrophoresis. Seventeen neighboring protein pairs have been identified. Eight are seen with at least two reagents: K-RS:p38, D-RS:K-RS, R-RS dimer, K-RS dimer, K-RS:Q-RS, E/P-RS:K-RS, E/P-RS:I-RS, and Q-RS with one of the nonsynthetase proteins. Nine more are observed with one reagent: D-RS dimer, R-RS:p43, D-RS:Q-RS, D-RS:M-RS, K-RS:L-RS, I-RS:R-RS, D-RS:E/P-RS, I-RS:Q-RS, I-RS:L-RS. One trimeric association is seen: E/P-RS:I-RS:L-RS. The observed neighboring protein pairs suggest that the polypeptides within the aminoacyl-tRNA synthetase complex are distributed in three structural domains of similar mass. These can be arranged in a U-shaped particle in which each “arm” is considered a domain and the third forms the “base” of the structure. The arms have been termed domain I (D-RS, M-RS, Q-RS) and domain II (K-RS, R-RS), with domain III (E/P-RS, I-RS, L-RS) assigned to the base. The smaller proteins (p38, p43) may bridge the domains. This proposed spatial relationship of these domains, as well as their compositions, are consistent with earlier studies. Thus, this study provides an initial three-dimensional working model of the arrangement of polypeptides within the multienzyme aminoacyl-tRNA synthetase complex.

Asialoorosomucoid (ASOR) was derivatized with 5 homobifunctional N-hydroxysuccinimide (NHS) ester crosslinkers. NHS/ASOR derivatives were synthesized, purified, and applied within 10 min to isolated rat hepatocytes at 4°. Specific binding of these 125I-labeled derivatives was ∼90% in the presence of either EGTA or excess ASOR. Specific crosslinking, assessed by the resistance of specifically bound NHS/125I-ASOR to release by EGTA, was 50-75% of the specifically bound ligand. The extent of specific crosslinking correlated with the average number of NHS groups per ASOR and was controlled by varying the molar ratio of crosslinker to ASOR during the synthesis. Crosslinking proceeded rapidly at 4° as a 1st-order process (k = 0.25 min-1, t1/2 = 2.8 min). After being crosslinked with any of the NHS/125I-ASOR derivatives, cells were washed with EGTA, solubilized in Triton X 100, and analyzed by SDS-PAGE and autoradiog. Major bands were observed at Mr ≃ 83, 94, and 105 kilodaltons corresponding to the expected size of 1:1 adducts between NHS/ASOR (Mr ≃ 41.3 kilodaltons) and the 3 subunits of the receptor, Mr ≃ 43, 50, and 60 kilodaltons. The 3 subunits, rat hepatic lectin (RHL) 1, 2, and 3, were labeled in the ratio of about 1.0:1.2:1.0, resp. After crosslinking, a polyclonal goat antibody to the receptor immunoprecipitated up to 100% of the specifically crosslinked NHS/125I-ASOR. Preimmune IgG immunoprecipitated <1% of the radiolabeled ligand. Cell surface receptors were crosslinked to NHS-ASOR, extracted with Triton X 100, immunoprecipitated with anti-orosomucoid-Sepharose, and subjected to Western blot anal. By use of antisera specific for RHL 1 or RHL 2/3 (from K. Drickamer), crosslinked complexes of Mr ∼85 kilodaltons or ∼90-115 kilodaltons, resp., were detected as were uncrosslinked native subunits. The ratio of free to crosslinked subunits was ∼10:1 for RHL 1 and ∼0.5:1 for RHL 2/3. Apparently, all 3 receptor subunits can crosslink to ligand. A model is proposed in which the native receptor is a heterohexamer composed of 4 subunits of RHL 1 and 2 subunits of RHL 2 and/or RHL 3.

Objective To explore the application timing of tromethamine in cesarean section of patients with central placenta previa. Methods Sixty-four women underwent cesarean section in Department of Obstetrics and Gynecol. in the hospital from Jan. 2013 to Dec. 2014, then they were diagnosed as central placenta previa and divided into observation group and control group based on random number table according to the sequence of admission, 32 women in each group. The patients in observation group were treated by tromethamine immediately after delivery of fetus, while the patients in control group were treated by tromethamine after delivery of placenta. The numbers of automatic placental expulsion, the numbers of manual removal of placenta, operation time, and the amounts of blood loss during operation in the two groups were compared. Results The amounts of blood loss during operation and at two hours after operation in observation group were statistically significantly less than those in control group (P<0.01). There was no statistically significant difference in the amount of blood loss within 24 h after operation between the two groups (P>0.05). The rate of automatic placental expulsion in observation group was statistically significantly higher than that in control group, while the rate of manual removal of placenta in observation group was statistically significantly lower than that in control group (P<0.01). The operation time in observation group was statistically significantly shorter than that in control group (P<0.01). Six patients in observation group were treated by uterine gauze, while fourteen patients in control group were treated by uterine gauze, there was statistically significant difference between the two groups (P<0.01). The rates of intraoperative and postoperative blood transfusion in observation group and control group were 15.6% and 37.5%, resp., there was statistically significant difference between the two groups (P<0.01). There was no statistically significant difference in the incidence rate of adverse reactions between the two groups (P>0.05). Conclusion Uterine body injection of tromethamine immediately after delivery of fetus during cesarean section of patients with central placenta previa can quickly promote lower segment uterine contraction, improve placental delivery rate, reduce the amount of blood loss during operation, shorten operation time, the adverse reactions are few, but the price is expensive, so it can be used as obstetric emergency medication or preventive medication of high risk pregnant women.

Purpose: The article explored the clin. effect of Hemabate combined with B-Lynch suture in the treatment of postpartum hemorrhage due to uterine asthenia. Methods: In the paper, 100 cases of postpartum hemorrhage due to uterine contraction and fatigue admitted to the Fuyang District Maternity and Child Health Hospital from March 2014 to March 2016 were selected as the research objects, and were divided into observation group and control group by random number method, with 50 cases in each group. The observation group was treated with Simabate combined with B-Lynch suture, and the control group was treated with traditional hemostasis. The clin. efficacy, operation time, intraoperative blood loss, 24 h postoperative blood loss, postoperative Hb level, hospital stay and postoperative complications were observed in the two groups. Results: The total effective rate of the observation group was 96.00%, and the control group was 80.00%. The difference between the two groups was statistically significant (P < 0.05); the operation time of the observation group was (49.98 ± 7.24) min, and the intraoperative blood loss was ( 514.43 ± 29.53) ml, 24 h postoperative blood loss was (125.97 ± 15.26) ml, hospitalization time was (7.92 ± 2.36) days, the control group was (71.41 ± 6.08) min, (720.16 ± 31.27) ml, (230.14 ± 21.91) ml, (10.04 ± 3.71)d, the difference between the two groups was statistically significant (P < 0.05). The Hb level of the observation group was higher than that of the control group, but the difference was not statistically significant (P > 0.05). The incidence of complications in the observation group was significantly lower than that in the control group, but the difference was not statistically significant (P > 0.05). Conclusion: The clin. effect of Simabate combined with B-Lynch suture in the treatment of postpartum hemorrhage due to uterine attenuation was definite and could shorten the operation time, to reduce intraoperative and postpartum hemorrhage, it was a fast, safe and easy way to stop bleeding, with fewer complications.

Objective To evaluate the preventive effect of dexmedetomidine on adverse reactions induced by carboprost tromethamine during cesarean section. Methods The pregnant women preparing for low-segment cesarean section were randomly divided into group D (dexmedetomidine), group O (ondansetron), and group C (control group), 30 women in each group. Cesarean section was completed in all the cases under continual epidural anesthesia, then they were treated by i.v. transfusion of 20 U oxytocin after delivery and deltoideus triangularis injection of 250 μg carboprost tromethamine. The cases in group D were treated by i.v. transfusion of dexmedetomidine (load capacity 1 μg/kg, the duration time of i.v. transfusion was less than 10 min, the infusion speed was 0.4 μg·kg-1·h-1) before 10 min of carboprost tromethamine treatment. The cases in group C were treated by i.v. injection of ondansetron after delivery. The cases in group C were treated by normal saline at equal quantity. Mean arterial blood pressure (MAP), heart rate, and SpO2 at 5 min after entering operation room (baseline, T0), 5 min after injection of carboprost tromethamine (T1), and the end of surgery (T2) were recorded. The incidence rates of nausea, vomiting, chest distress, and flushed face from usage of carboprost tromethamine to end of surgery were calculated Modified OAA/S scoring was conducted at T2 and after 2 h of surgery (T3). Results Compared with group D, MAP at T2 and T3 in group C and group O increased significantly, heart rates at T2 and T3 in group C and group O fastened significantly, SpO2 at T2 and T3 in group C and group O decreased significantly, there were statistically significant differences (P<0.05). Compared with group C, the incidence rates of chest distress, flushed face, nausea, and vomiting in group D decreased significantly, the incidence rates of nausea and vomiting in group O decreased significantly, there were statistically significant differences (P<0.05). Compared with group C, OAA/S score in group D increased significantly (P<0.05). Conclusion I.v. transfusion of dexmedetomidine before carboprost tromethamine during cesarean section can prevent gastrointestinal and cardiovascular reactions caused by carboprost, the effect is superior to ondansetron.

Objective: To explore the clin. efficacy and safety of carboprost tromethamine combined with low B-Lynch suture in the treatment of refractory placenta previa and postpartum hemorrhage (PPH). Methods: Using retrospective research methods, 108 patients with refractory placental previa PPH admitted from May 2011 to May 2015 were divided into control group (n=53) and observation group (n=55) according to the treatment method. . Both groups were given cesarean section for delivery. The control group was given oxytocin, and the placenta was dissected with 8 intermittent suture. The observation group was treated with carboprost tromethamine combined with low B-Lynch suture. The clin. efficacy, intraoperative blood loss, vaginal bleeding 2h postoperatively, vaginal bleeding 24h postoperatively, and the incidence of adverse reactions were compared between the two groups. If the hemostasis is ineffective, the two groups of patients will be treated with catheter arterial embolization (TAE) to stop the bleeding. If the TAE fails to stop the bleeding, hysterectomy will be performed. Results: (1) After treatment, the number of markedly effective, effective and ineffective cases in the observation group were 34, 19 and 2 resp., and the total clin. effective rate was 96.36% (53/55), the number of markedly effective, effective and ineffective cases in the control group were resp. 15 cases, 24 cases and 14 cases, and the total clin. effective rate was 73.58% (39/53). The difference in efficacy between the two groups was statistically significant (P<0.05). (2) There was no statistically significant difference in intraoperative blood loss between the two groups (P>0.05), but the vaginal bleeding in the observation group was significantly less than that in the control group at 2h and 24h after surgery (P<0.01). (3) The incidence of adverse reactions after treatment in the control group was 22.64% (12/53), and the observation group was 7.27% (4/55). The difference in the incidence of adverse reactions between the two groups was statistically significant (P<0.01). Conclusion: Carboprost tromethamine combined with low B-Lynch suture in the treatment of refractory placenta previa postpartum hemorrhage has a significant clin. effect and a low incidence of postoperative adverse reactions.

Methods: 114 cases of high-risk parturient women were divided into group A and group B, which received carboprost trometamol and misoprostol after cesarean section, resp. Results: There was no significant difference of postpartum hemorrhage rate between the two groups, but the 2 h hemorrhage and 24 h hemorrhage in group A were significantly lower than those in group B. Conclusion: Carboprost trometamol shows a better preventive effect on hemorrhage after cesarean section than misoprostol.
