Pediatric patients, especially those with rare diseases, represent a population that has a high tendency towards offlabel drug use (OLDU) and needs a more careful practice of pharmacotherapy than in adults. We aimed to investigate biotechnol. drug use in children with rare diseases requiring OLDU. This retrospective study examined all single-diagnosed OLDU applications (n = 5792) for 4992 children (<18-yr) in Turkey. Applications of rare diseases were selected, and their descriptive characteristics were examined, including demog. features of patients, biotechnol. drug utilization status, and disease categories. The off-label statuses of the drugs at the end of 2020 were also examined In total, 77.7% (n = 4501) of OLDU applications were made for rare diseases. Biotechnol. drug use was higher in rare disease applications than in nonrare diseases (37.9% vs. 19.2%, resp.; p < 0.0001). Canakinumab was the top applied biotechnol. drug (73.2%). Compared to that in small-mol. drugs, the mean age of patients was higher in biotechnol. drug-containing applications (8.1 ± 5.3 vs. 9.7 ± 4.9, resp.; p < 0.0001). Biotechnol. drug use was higher in nonneoplastic rare diseases (40.3%) than in neoplastic rare diseases (26.4%), (p < 0.0001). At the end of 2020, the approval status of the off-label indications covered in 2016 was significantly higher for rare (24.4%) vs. nonrare (5.2%, p < 0.0001) diseases and for biotechnol. (32.3%) vs. smallmol. (13.9%, p < 0.0001) drugs. In total, 87.7% of the drugs would have to be still used in the off-label setting at the end of 2020. It was seen that more than three-quarters of the pediatric OLDU applications are for rare diseases, and the need for biotechnol. OLDU in this group is almost 2-fold of small-mol. drug use. While further projected findings imply a higher approval tendency for rare diseases and biotechnol. drugs, there seems to be more room for improvement for pediatric drug use. A total of 186 MCRC patients aged =65 years, receiving chemotherapy between Jan. 1, 2004 and Oct. 1, 2015 were identified. Pretreatment levels of carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA 19-9), LDH, CRP and NLR were measured. Effect of these inflammatory factors on overall survival (OS) and first-line progression-free survival were analyzed. Median PFS and OS were 6.70 mo and 25.62 mo. CEA =85 ng/mL (P=0.010), CA 19-9 =32.97 U/mL (P=0.010), LDH =325 U/L (P=0.015), CRP =11 mg/L (P=0.004) and NLR =2.12 (P=0.045) were associated with poor OS. Furthermore, LDH (P=0.025) was demonstrated as an independent prognostic factor of OS for all patients, so did the combination of CEA with CA 19-9 (P=0.009). When predicting the one, three and five-year survival, combination of CEA with CA 19-9 had higher sensitivity compared with CEA alone or CA 19-9 alone resp. For right-sided colon cancer, CEA (P<0.001) and CA 19-9 (P=0.003) were related with OS and CEA (P=0.002) was the independent prognostic factor. For left-sided colorectal cancer, inflammatory factors related with OS were LDH (P=0.039), CRP (P=0.004) and NLR (P=0.020) and CRP (P=0.040) was the independent prognostic factor. However, no inflammatory factors were contained in multivariate COX regression. CEA, CA 19-9, LDH, CRP and NLR were related with OS in geriatric patients with MCRC. Right-sided and left-sided groups had different independent prognostic markers, CEA and CRP resp. This study aimed to investigate the impact of the inferior mesenteric artery (IMA) lymph node metastasis [IMALN (+)] on prognosis in left-sided colorectal cancer (LCRC). A total of 285 patients with stage III LCRC and 118 patients with stage IV LCRC who underwent resection of primary tumor between 2005 and 2016 were included. IMALN (+) patients (n = 10) had worse overall survival (OS) than patients without IMA lymph node metastasis [IMALN (-); n = 275] in stage III LCRC (p = 0.007). Multivariate anal. revealed that IMALN (+) was a prognostic factor in stage III LCRC (OS, HR = 3.09, p = 0.043). Conversely, there was no difference between the OS of IMALN (+) and stage IV LCRC with distant lymph node metastasis only [stage IV LCRC (LYM); n = 21; p = 0.434]. The prognosis of IMALN (+) was worse than that of IMALN (-); it was similar to that of stage IV LCRC (LYM).

A round robin comparison was performed in order to test the performance of a recently developed LC-MS/MS method for quantification of 6 folate forms. Eighty-nine samples representing the food groups of fruits, vegetables, legumes, cereals, dairy products, meat, and offal were analyzed by two LC-MS/MS methods and a microbiol. assay (MA). The 3-yr disease-free survival rate of the twice-daily regimen was not inferior to that of the conventional three-times-daily regimen, and the twice-daily regimen did not lead to an increase in adverse events. We conducted a multicenter randomized trial to assess the noninferiority of a twice-daily compared with a three-times-daily UFT/LV regimen for stage II/III colorectal cancer in an adjuvant setting. Patients were randomly assigned to group A (three doses of UFT [300 mg/m2 per day]/LV [75 mg per day]) or B (two doses of UFT [300 mg/m2 per day]/LV [50 mg per day]). The primary endpoint was 3-yr disease-free survival. In total, 386 patients were enrolled between July 28, 2011, and Sept. 27, 2013. Compared with the non-antibiotic group, antibiotic exposure was inversely associated with the mortality in the antibiotic group after adjustment for demog. and other potential confounders (a history of medication: HR, 0.650(95%CI: 0.360 to 1.173); an increase per day: HR, 0.967(CI: 0.924 to 1.011); 1-6 days: HR, 0.859(CI: 0.441 to 1.674); 7-40 days: HR, 0.474(CI: 0.225 to 0.999); P for trend = 0.040). A test for the interaction between sex was statistically significant (p = 0.016). A similar result was found as measured by landmark and PSM anal. Significant neg. dose-response relationship was shown by smooth curve anal. in the male patients but not female after adjustment for confounders(p = 0.028). No association was found between the antibiotic medication and adverse events of bevacizumab. We show that with the optimized nanopillar-assisted separation (NPAS) method using surface-enhanced Raman scattering, we were able to measure MTX in PBS and serum in the linear range of 5-150μM and confirmed that MTX detection can be carried out even in the presence of LV. Addnl., when NPAS was combined with centrifugal filtration, a quantification limit of 2.1μM for MTX in human serum sample was achieved. The developed detection method enables fast detection (10 min) and quantification of MTX from human serum (>90% accuracy). Furthermore, we show the potential of the developed method for TDM, when quantifying MTX from clin. samples, collected from patients who are undergoing high-dose MTX therapy.

Primary cell cultures are challenging, but reliable model reflecting tumor response in vitro. The study was designed to examine if the increased electropermeabilization can overcame initial drug insensitivity in chondrosarcoma cells from lung metastasis. We established a primary cell culture and evaluated the cytotoxic impact of four drugs-cisplatin (CDDP), camptothecin, 2-methoxyestradiol, and leucovorin calcium (LeuCa). After determination of parameters allowing for electropermeabilization, we performed electrochemotherapy in vitro with the least toxic drugs-CDDP and LeuCa. Although combining CDDP and leucovorin together increased their toxicity and supported apoptosis, application of pulsed elec. fields (PEFs) brought no advantage for their efficacy. The study emphasizes the need for introduction of primary cell cultures into studies on pulse elec. fields as model frequently less sensitive to PEF-based treatments than continuous cell lines.

Limited literature is available for bevacizumab exposure-response relationship and there is not a concentration threshold associated with an optimal disease control. This prospective observational study in patients with metastatic colorectal cancer (mCRC) aims to evaluate, in a real-life setting, the relationship between bevacizumab through concentrations at steady state (Ctrough, SS) and disease control. Ctrough, SS were drawn, coinciding with the radiol. evaluation of the response (progression or clin. benefit). Generalized estimating equations (GEE) anal. was performed. To test the association between Ctrough, SS in each patient with overall survival (OS) or progression-free survival (PFS), Cox proportional hazard models were developed. Data included 50 bevacizumab Ctrough, SS from 27 patients. The GEE model did not suggest any pos. association between bevacizumab Ctrough, SS and clin. benefit (OR 0.99, 95% CI: 0.98-1.02, p = 0.863). The Cox regression showed association between higher median Ctrough, SS with better OS (HR 0.86, 95% CI: 0.73-1.01, p = 0.060), but not with PFS. We cannot confirm a relationship between bevacizumab Ctrough, SS and clin. benefit but this is the first real-world study trying to show a relationship between bevacizumab Ctrough, SS and disease control in mCRC. It was conducted in a small sample size which reduces the level of evidence. Further controlled randomized studies with a sufficient number of patients are required.
