The polyol isomalt (Palatinit) is a well established sugar replacer. The impact of regular isomalt consumption on metabolism and parameters of gut function in nineteen healthy volunteers was examined in a randomized, double-blind, cross-over trial with two 4-wk test periods. Volunteers received 30 g isomalt or 30 g sucrose daily as part of a controlled diet. In addition to clin. standard diagnostics, biomarkers and parameters currently discussed as risk factors for CHD, diabetes or obesity were analyzed. Urine and stool Ca and phosphate excretions were measured. In addition, mean transit time, defecation frequency, stool consistency and weight were determined Consumption of test products was affirmed by the urinary excretion of mannitol. Blood lipids were comparable in both phases, especially in volunteers with hyperlipidemia, apart from lower apo A-1 (P = 0·03) for all subjects. Remnant-like particles, oxidized LDL, NEFA, fructosamine and leptin were comparable and not influenced by isomalt. Ca and phosphate homeostasis was not affected. Stool frequency was moderately increased in the isomalt phase (P = 0·006) without changes in stool consistency and stool water. This suggests that isomalt is well tolerated and that consumption of isomalt does not impair metabolic function or induce hypercalciuria. In addition, the study data indicate that isomalt could be useful in improving bowel function.

The polyol isomalt (Palatinit) is a very low glycemic sugar replacer. The effect of food supplemented with isomalt instead of higher glycemic ingredients like sucrose and/or starch hydrolyzates on metabolic control in patients with type 2 diabetes was examined in this open study. Thirty-three patients with type 2 diabetes received a diet with foods containing 30 g/d isomalt instead of higher-glycemic carbohydrates for 12 wk. Metformin and/or thiazolidindiones were the only concomitant oral antidiabetics allowed during the study. Otherwise, the participants maintained their usual diet during the test phase, but were instructed to refrain from addnl. sweetened foods. Before start, after 6 wk and 12 wk (completion of the study), blood samples were taken and analyzed for clin. routine parameters, metabolic, and risk markers. Thirty-one patients completed the study. The test diet was well accepted and tolerated. After 12 wk, significant reductions were observed for: glycosylated Hb, fructosamine, fasting blood glucose, insulin, proinsulin, C-peptide, insulin resistance (HOMA-IR), and oxidized LDL (an atherosclerosis risk factor). In addition, significant lower nonesterified fatty acid concentrations were found in female participants. Routine blood measurements and blood lipids remained unchanged. The substitution of glycemic ingredients by isomalt and the consequent on reduction of the glycemic load within otherwise unchanged diet was accompanied by significant improvement in the metabolic control of diabetes. The present study is in agreement with findings of previous reported studies in human subjects demonstrating beneficial effects of low glycemic diets on glucose metabolism in patients with diabetes mellitus type 2.

The objective of this study was to produce a frozen yogurt containing low fat and no added sugar. Samples containing 5% polydextrose, 0.065% aspartame and acesulfame-K mixture, and different levels of inulin and isomalt (5.0, 6.5, and 8.0%) were produced at pilot scale and analyzed for their phys. and chem. properties including proximate composition, viscosity, acidity, overrun, melting rate, heat shock stability, as well as sensory characteristics, and viability of lactic acid bacteria. With the addition of inulin and isomalt, viscosity increased by 19 to 52% compared with that of sample B (reduced-fat control). The average calorie values of samples substituted with sweeteners were about 43% lower than that of original sample. Low-calorie frozen yogurt samples melted about 33 to 48% slower than the reduced-fat control sample at 45 min. Based on quant. descriptive profile test results, statistically significant differences among products were observed for hardness, iciness, foamy melting, whey separation, and sweetness characteristics. The results of principal component anal. showed that the sensory properties of the sample containing 6.5% inulin and 6.5% isomalt were similar to those of control. Lactic acid bacteria counts of frozen yogurt were found to be between 8.12 and 8.49 log values, 3 mo after the production The overall results showed that it is possible to produce an attractive frozen yogurt product with the incorporation of inulin and isomalt with no added sugar and reduced fat.

We performed a comprehensive search of medical bibliog. databases to identify interventional studies reporting the effect of probiotics (or synbiotics) supplementation on biomarkers of oxidative stress. Twenty seven articles that included 1363 subjects (709 cases and 699 controls) were included to our analyses. Oxidative stress parameters levels, including total antioxidant capacity (TAC) (0.31 mmol/L, 95% CI: 0.13-0.50, P < 0.001), glutathione (GSH) (SMD: 0.44 μmol/L, 95% CI: 0.26-0.62, P < 0.001), superoxide dismutase (SOD) (SMD: 0.48 U/mg, 95% CI: 0.16-0.81, P = 0.004) and nitric oxide (NO) (SMD: 0.57 μmol/L, 95% CI: 0.19-0.94, P = 0.003) were higher in probiotics (or synbiotics) group compared to controls. Moreover, malondialdehyde (MDA) (SMD = -0.45 μmol/L, 95%CI = -0.63 to -0.26, P < 0.001) level was lower than controls. Probiotics and synbiotic supplementation improve antioxidant resistance and increase the amount of antioxidant enzymes in the body. A novel method is herein proposed, based on the combination of gas foaming and fiber templating. In particular, the tubular pores have been obtained by leaching of melt-spun saccharide fibers (cotton candy) within CO2-foamed poly(ε-caprolactone). The optimization of the scaffold has been conducted via the selection of the proper saccharide, its thermal treatment to stabilize the spun fiber (crystallization), the formation of the polymer-fiber composite, the selection of the proper gas-foaming processing parameter and the final leaching in water.

Continuous production of directly compressible powders was achieved by coprocessing acetaminophen and carbohydrates via spray drying. Binary and ternary powder mixtures containing drug substance and carbohydrates were prepared by co-spray drying and evaluated on spray drying processibility, powder hygroscopicity, flowability, and compactability. The influence of process parameters during spray drying on the compaction behavior of drug/excipient mixtures was investigated via Heckel anal. Erythritol, lactose, maltodextrin, and mannitol were efficient in co-spray drying with acetaminophen. However, lactose mixtures showed poor flowability. Spray dried mixtures containing mannitol and erythritol were characterized as non-hygroscopic, highly dense, and good flowing powders. Mannitol increased tablet tensile strength in contrast with the poor compactability of erythritol. Maltodextrin was selected for further experiments because it provided excellent tablet tensile strength. The use of erythritol, maltodextrin and mannitol in binary drug/excipient mixtures resulted in high process yields. Compacts of erythritol, mannitol, and maltodextrin were characterized by higher tablet tensile strength at higher spray drying temperatures due to the increased particle fragmentation of erythritol and mannitol mixtures and to the increased plastic deformation of maltodextrin formulations. A combination of erythritol, maltodextrin, and mannitol was selected for further formulation and process optimization of co-spray dried powders for direct compression.
