Objective: To analyze the preventive effect of the combination of carboprost tromethamine and oxytocin on postpartum hemorrhage in cesarean section of high-risk women, and observe the occurrence of adverse reactions after maternal medication. Methods: A total of 120 high-risk pregnant women who were scheduled for cesarean delivery at the Obstetrics Department of Liaobu Hospital in Dongguan City were collected, and randomly divided into 3 groups. 40 cases in group A, who were given oxytocin alone. 40 patients in group B were treated with oxytocin combined with carboprost methylate suppository. 40 patients in group C were given oxytocin combined with carboprost tromethamine. The intraoperative and postoperative bleeding volume, postpartum hemorrhage, treatment and adverse reactions of the three groups were compared. Results: The intraoperative and postoperative 2 h bleeding volume of group B and group C were significantly lower than that of group A (P < 0.05), among which the difference of group C was more significant. The bleeding volume in group C was significantly lower than that in group A and group B at 2-24 h after operation (P < 0.05). The incidence of postpartum hemorrhage, blood transfusion rate, and treatment rate of other hemostatic measures in group B and group C were significantly lower than those in group A (P < 0.05). Conclusion: The combination of carboprost tromethamine and oxytocin can effectively reduce the amount of postpartum hemorrhage and the incidence of postpartum hemorrhage in high-risk women, and it is safe and worthy of further promotion.

The postpartum hemorrhage rate and the amount of bleeding were compared between the two groups. Results: There was no significant difference in the postpartum hemorrhage rate between the 2 groups (P > 0.05). The amount of bleeding at postpartum 2 h and 24 h in group A were less than those of group B (P < 0.05). No significant difference was found in the incidence of adverse reactions between the 2 groups (P > 0.05). Methods One hundred and twenty-eight nulliparous parturients who were at full term with a singleton fetus, of American Society of Anesthesiologists phys. status I or II, aged 24-37 yr, weighing 54-78 kg, scheduled for elective caesarean section under combined spinal-epidural anesthesia, were randomly divided into 4 groups (n = 32 each) using a random number table: control group (group C) and different concentrations of propofol groups (P1-3 groups). Carboprost tromethamine 250 μg was injected into the body of the uterus, and propofol with the target plasma concentrations of 0.8, 1.2 and 1.6 μg/mL was simultaneously given by target-controlled infusion in P1, P2 and P3 groups, resp., and normal saline was infused at a rate of 0.5 mL·kg-1·h-1 in group C. The occurrence of cardiovascular events was recorded from the end of carboprost tromethamine administration until the end of surgery. The related adverse effects after carboprost tromethamine administration and Ramsay sedation score at 15 mm after carboprost tromethamine administration were recorded, and satisfactory sedation was defined as Ramsay sedation score of 2. The occurrence of complications associated with combined spinal-epidural anesthesia was recorded during the postoperative follow-up. Results Compared with group C, the incidence of carboprost tromethamine-related adverse effects was significantly decreased in P2 and P3 groups, the rate of satisfactory sedation was significantly increased in P1 and P2 groups, the incidence of hypotension and tachycardia was significantly decreased in group P1 (P<0.05), and no significant change was found in the incidence of carboprost tromethamine-related adverse effects in group P1 and in the rate of satisfactory sedation in group P3 (P>0.05). Compared with group P1, the incidence of carboprost tromethamine-related adverse effects was significantly decreased in P2 and P3 groups, the rate of satisfactory sedation was significantly increased in group P2, and the rate of satisfactory sedation was significantly decreased in group P3 (P<0.05). Compared with group P2, the rate of satisfactory sedation was significantly decreased (P<0.05), and no significant change was found in the incidence of carboprost tromethamine-related adverse effects in group P3 (P>0.05). No cardiovascular events were found in group P2, and the incidence of hypotension was 9% in group P3. Conclusion The optimum target plasma concentration of propofol in preventing the adverse effects of carboprost tromethamine is 1.2 μg/mL in the patients undergoing caesarean section.

Objective: To compare the efficacy and side effects of 0.2 mg methyl-ergometrine IM, 400 μg misoprostol sublingual and 125 μg 15 Me PGF2α IM in active management of third stage of labor. Method: Two hundred low risk pregnant women with induced or spontaneous labor were randomized to receive either 400 μg misoprostol sublingually or 0.2 mg methyl-ergometrine i.m. or 125 μg 15-Me PGF2α i.m., after the delivery of anterior shoulder of baby. The main outcome measures were: blood loss more than 500 mL, need for addnl. oxytoxic drug, change in Hb level and side effects due to drugs. Results: The median estimated blood loss, blood loss more than 500 mL, need for addnl. oxytocic drug and change in Hb levels were similar in all three groups. The significant side effects in the misoprostol group were shivering, pyrexia (temperature > 38°C) and vomiting, which were self-limiting. Diarrhea was significantly more in the 15 Me PGF2α group. Three women in methyl-ergometrine group underwent manual removal of placenta. One woman in misoprostol group received blood transfusion. Conclusion: Sublingual misoprostol appears to be as effective as i.m. methyl-ergometrine and i.m. 15-Me PGF2α in the prevention of postpartum hemorrhage. It can be a good alternative in resource poor setting.

Objective: To investigate application of carboprost tromethamine in treatment of hypertension in pregnancy. Methods: 200 cases of patients with hypertension in pregnancy were selected as a treatment group, and 200 cases of normal pregnancy at the same period were selected as a control group. Two groups underwent lumbar epidural anesthesia for cesarean section surgery, the control group was instantly given oxytocin 20 IU injection for uterine wall after childbirth of fetus, the treatment group was one-time given carboprost tromethamine injection 250 μg after childbirth of fetus, and both groups were given 20 IU of oxytocin via continuous i.v. infusion for 6 h. The bleeding state, incidence of postpartum hemorrhage, Hb level, and blood pressure after childbirth were observed during surgery and after surgery within 2 h and 24 h. Results: Compared with the control group, the amount of bleeding within 2h, 24 h after surgery of the treatment group was lower, and incidence of postpartum hemorrhage was lower (P < 0.05). Blood pressure difference of the two groups was not statistically significant (P > 0.05) and the two groups had no significant adverse reactions. Conclusions: Carboprost tromethamine is an effective measure to prevent postpartum hemorrhage of hypertension in pregnancy.

Dexmedetomidine prevented Hemabate adverse reactions in cesarean section was studied. After Hemabate in cesarean section, D group treated dexmedetomidine and C group was control group. BP, HR, RR, SpO2 were recoreded at room (T0), before Hemabeta (T1), after drug 10 min (T2). At T2, BP and HR were higher than T0, SpO2 of group D change was small, SpO2 of group C decreased. Adverse reaction of group D was lower than group C. Dexmedetomidine reduced Hemabate adverse reactions in cesarean section.
