Motherwort (YiMuCao), a traditional Chinese herb, has been shown beneficial effects for women’s diseases. This meta-anal. aimed to evaluate the efficacy and safety of motherwort injection add-on therapy to carboprost tromethamine for prevention of post-partum blood loss. A systematic literature search was conducted in PubMed, Embase, Cochrane Library, CNKI, VIP and Wanfang from their inception to Dec. 2017. Randomized controlled trials that determined the add-on effects of motherwort injection to carboprost for prevention of post-partum blood loss were eligible. Pooled risk ratio (RR) and mean difference (MD) with 95% confidence interval (CI) were used to summarize the effect sizes. Eight trials including 1276 pregnant women fulfilled the inclusion criteria. Prophylactic use of motherwort injection add-on therapy significantly reduced the post-partum 2 h (MD -127.5 mL; 95% CI -149.13 to -105.88) and 24 h (MD -146.85 mL; 95% CI -179.77 to -113.94) blood loss and incidence of post-partum hemorrhage (RR 0.28; 95% CI 0.17-0.45) than carboprost. Moreover, adjunctive treatment with motherwort injection significantly decreased the length of the third stage of labor (MD -3.41 min; 95% CI -4.33 to -2.49) and duration of lochia (MD -7.13 days; 95% CI -8.49 to -5.76). There was no statistical significant difference in the incidence of adverse events (RR 0.76; 95% CI 0.50-1.16). VProphylactic use of motherwort injection add-on therapy to carboprost tromethamine could reduce post-partum blood loss. However, more well-designed trials are necessary to confirm the findings of this study due to the methodol. flaws of the included trials.

Objective To observe the preventive effect of Guyuan Fujiu decoction combined with carboprost tromethamine on postpartum hemorrhage caused by uterine inertia in vaginal delivery. Methods A total of 110 cases of postpartum hemorrhage due to uterine inertia in the hospital from Apr. 2018 to June 2020 were randomly divided into control group and observation group, 55 cases in each group. The control group was given carboprost tromethamine injection after the delivery of the fetus, and the observation group was given Guyuan Fujiu decoction on the basis of the control group. The incidence of postpartum hemorrhage, amount of postpartum hemorrhage, hemostatic time, use of other hemostatic measures, changes of Hb and hematocrit were compared between the two groups. Results The postpartum hemorrhage rate of the observation group (3.64%) was significantly lower than that of the control group (14.55%), the difference was statistically significant (χ2=3.960, P<0.05). Postpartum hemostasis time [(23.56±3.14) min] was significantly shorter than that in the control group [(40.05±5.74) min], the difference was statistically significant (t=18.691, P<0.05). Compared with the control group, the amount of bleeding in the observation group decreased significantly at 30 min, 2 h and 24 h postpartum (t=8.654, 13.495 and 15.496, P<0.05). The application rate of other hemostatic measures in the observation group (7.27%) was significantly lower than that in the control group (21.82%) (χ2=4.681, P<0.05). Compared with the control group, the decrease of maternal Hb and hematocrit in the observation group were significantly reduced (t=22.817, 28.939, P<0.05). Compared with the control group, the serum D-dimer level in the observation group was significantly lower at 2 h postpartum, and the FIB level was significantly higher (t=19.492, 17.256, P<0.05). The level of serum antithrombin-III in the observation group was significantly higher than that in the control group (t=6.090, 10.092, P<0.05). The adverse reaction rate of the observation group (7.27%) was lower than that of the control group (16.36%), but the difference was not statistically significant (χ2=2.181, P>0.05). Conclusion Guyuan Fuguo decoction combined with carboprost tromethamine can be used to prevent uterine hypostasis postpartum hemorrhage, reduce postpartum hemorrhage, improve coagulation function, promote uterine contraction and drug safety.

Objective: To observe the effect of carboprost tromethamine (hemabate) and oxytocin in cesarean section for preventing and reducing the intraoperative bleeding and compare their adverse reaction. Methods: From Nov. 2013 to March 2014, a total of 40 parturient women underwent cesarean section were included in the study. They were randomly divided into group I (20 cases using carboprost tromethamine injection) and group II (20 cases using oxytocin) according to different drugs for the injection into their uterine corpus immediately after fetal disengagement. There were no significant differences in maternal age, gestational age, body weight in general between two groups (P > 0.05). The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Shanghai First Maternity and Infant Hospital. Informed consent was obtained from each participants. Results: The amount of blood loss during cesarean section in group I was obviously lower than that of group II (P < 0.05). There were significant differences in vital signs at 10 min after treatment between two groups (P < 0.05). The incidence rate of adverse reaction in group I was significantly higher than that of group II (P < 0.01). Conclusion: Carboprost tromethamine injection in caesarean operation can effectively decrease the intraoperative bleeding, but more attention should be paid on its adverse reaction.

A new simple, precise, accurate and rapid method was developed for determination of Carboprost Tromethamine from its pharmaceutical dosage form. The separation was carried out on a Waters C18 column (Symmetry -C18, 3.5μm, and 4.6×100mm) in isocratic mode with mobile phase comprising acetate buffer pH 3.7 and methanol (30:70, %volume/volume) by using Agilent 1100 series HPLC system with Agilent Chemstation software. The flow rate was 1 mL/min and Ultra-Violet detection was carried out at 200 nm. Every part of determination was performed at ambient column temperature The retention time was 6.827 min for Carboprost Tromethamine. The developed method was validated for parameters like specificity, accuracy, precision, robustness as per International Conference on Harmonization guidelines. Linearity for Carboprost Tromethamine was in the range of 83-249μg/mL and Correlation coefficient was found to be 0.999. The percentage recovery was found to be in the limit of 98.4-99.3 %. Statistical anal. of the results has been carried out revealing high accuracy and good precision. Hence this method can be of use and value for the quality control department of pharmaceutical companies manufacturing these formulations without any interference due its sensitivity, simplicity and selectivity.

Objective: To establish an HPLC-UV-CAD method for the simultaneous determination of carboprost, tromethamine and benzyl alc. in carboprost tromethamine injection. Methods: Carboprost, tromethamine and benzyl alc. were separated on a CAPCELL PAK CR 1:4 column (150 mm×4.6 mm, 5 μm). The mobile phase was composed of ultrapure water, 0.1 mol·L-1 ammonium acetate buffer (pH 6.2) and methanol with gradient elution mode at a flow about 1.0 mL·min-1; the injection volume was 10 μL and column temperature was 40 °C. CAD was used to detect carboprost and tromethamine, in which nebulization temperature was 35 °C and collection frequency was 30 Hz. UV was used to detect benzyl alc., in which detection wavelength was set at 258 nm and collection frequency was 5 Hz. Results: The linear range of carboprost was 9.38-56.2 μg·mL-1 (r = 0.997 1, n = 6); the lowest limit of detection was 0.3 μg·mL-1; LOQ was 0.6 μg·mL-1; average recovery was 104.7%. The linear range of tromethamine was 5.19-23.3 μg·mL-1 (r = 0.999 2, n = 6); the lowest limit of detection was 1 μg·mL-1; LOQ was 2.5 μg·mL-1; average recovery was 95.5%. The linear range of benzyl alc. was 0.297-1.78 mg·mL-1 (r = 1, n = 6), the lowest limit of detection was 5.0 μg·mL-1; LOQ was 10.0 μg·mL-1; average recovery was 100.1%. The actual measurement results of the sample were consistent with the standard method currently used. Conclusion: The HPLC method established does not require derivative operations, and is cheap, sensitive, simple, accurate, reproducible and accurate. This method can simultaneously detect carboprost, tromethamine and benzyl alc. in carboprost tromethamine injection.

To investigate the effect of carboprost tromethamine on post-partum hemorrhage associated with caesarean section. One hundred patients with postpartum hemorrhage induced by cesarean section who were admitted to Binzhou People’s Hospital from Oct. 2016 to August 2018 were selected. They were randomly assigned to two groups: control group and treatment group. Patients in the control group were administered oxytocin, while those in the treatment group received oxytocin in combination with carboprost tromethamine. The incidence of hemorrhage and adverse reactions were compared for the two groups. The two groups showed gradually increasing degrees of postpartum hemorrhage within 24 h, but the treatment group had significantly lower volume of postpartum hemorrhage than the control group at different time points (p < 0.05). At the 1st, 3rd and 5th days after delivery, the height of the uterine fundus decreased gradually in the two groups, but was smaller in the treatment group than in the oxytocin group at all time points (p < 0.05). Total response in the treatment group was 98%, which was significantly higher than 78% in the control group (p < 0.05). The incidence of adverse reactions in the treatment group (12%) was significantly lower than that in the control group (24%, p < 0.05). Carboprost tromethamine prevents postpartum hemorrhage after cesarean section. It effectively reduces bleeding and promotes uterine involution, without obvious adverse reactions.

Methods: 96 cases of vaginal delivery subsequently pregnant women after cesarean parturition were randomly divided into observation group of 46 cases and control group of 50 cases, and the observation group was given oxytocin 20U by i.v. infusion after delivery and Hemabate 250μg by deep i.m. injection in hip, while the control group was given oxytocin 20U by i.v. infusion after delivery. Postpartum hemorrhage between the groups and postpartum hemorrhage amount in 2h were compared. Results: Observation group had one case of postpartum hemorrhage (2.17%), and control group had 6 cases of postpartum hemorrhage in (12.00%), and the two groups showed no significant difference (P > 0.05 ). Observation group had decrease in average 2h postpartum hemorrhage amount than the control group (207.34 ± 78.88) vs (263.64 ± 87.49) ml, and the difference was statistically significant P < 0.05. Conclusions: Hemabate can be used for preventing vaginal delivery postpartum hemorrhage in subsequent pregnancy after cesarean parturition with high safety, to some extent, can reduce the incidence of postpartum hemorrhage.

The objective of this paper was to explore the applied opportunity and economical efficiency of carboprost tromethamine in cesarean section. 156 Cases were treated by carboprost tromethamine in cesarean section, some of them used carboprost tromethamine for prophylactic treatment before operation, and the others used carboprost tromethamine after operation under different opportunities. All the cases were treated by 20-40 U oxytocin at the same time. Some cases needed more than 250 μg carboprost tromethamine, and 15 min interval between each injection was necessary. The uterus became more harder, the hemorrhage volume became fewer after injection of carboprost tromethamine, 250 μg was ED for 86% of the cases, and 500 μg were ED for 96% of the cases. The adverse reaction included facial blushing, nausea and vomiting. Carboprost tromethamine could prevent and decrease the hemorrhage volume, and the risk of blood transfusion under postpartum hemorrhage.

Hemostatic effect of carboprost tromethamine in laparoscopic myomectomy was discussed. One hundred patients with uterine fibroids were randomly divided into treatment group and control group, they were injected with 1 mL carboprosttromethamine (250 μg) and oxytocin 20 U during operation to prevent bleeding. Bleeding amount, operation time and abdominal drainage in treatment group were less than those in control group (P<0.05). Temperature recovery time and evacuation time by anus had no statistical significance. Application of carboprost tromethamine in laparoscopic myomectomy could significantly decrease bleeding time, shorten operation time, with less adverse reaction and worth popularizing.
