Objective: To observe the effect and safety of hemabat (H) on prevention of postpartum hemorrhage in cesarean section and after cesarean section in high risk pregnant women. Methods: Four hundred and sixty-nine pregnant women with high hemorrhagic risk factors including twin pregnancy, polyhydramnios, fetal macrosomia, placenta previa were planned cesarean section. A total of 457 pregnant women were divided into 3 groups by operation indications. There were 239 cases of fetal macrosomia, 145 cases of twin pregnancy and polyhydramnios, and 73 cases of placenta previa. Three kinds of hysterotonics were used randomly in each group. Group oxytocin (O): 20 U oxytocin injected into the uterine plus 20 U oxytocin intravascularly, 152 women; Group oxytocin + hemabate (O + H): 20 U oxytocin and 250 μg hemabat injected into the uterine, 192 women; group H: 250 μg hemabat, injected into the uterine, 125 women. The amount of bleeding during the operation and within 2-h after delivery were measured. The side effect of each group was observed Results: The amount of bleeding during cesarean section in group O was (445 ± 262) ml, in group 0 + H (332 ± 218) ml, and in group H (375 ± 265) ml. There was an extremely significant difference between group O and group O + H (P < 0.01). The amount of bleeding within 2 h after delivery in group O was (176 ± 193) ml, in group O + H was (110 ± 114) ml, and in group H was (124 ± 103) ml. There was a significant difference between groups O, O + H and H. Among the 469 women, 31 had total amount of bleeding more than 1000 mL during operation and within 2 h after delivery, 48% (15 women) were in group O, 23% in group O + H and 29% in group H. The total amount of bleeding in group O was much more than group O + H and group H in the group of fetal macrosomia (P < 0.01, P < 0.01). Similar results were found in the group of twin pregnancy and polyhydramnios (P < 0.01, P < 0.01). The total amount bleeding in group O + H was much less than group O in the group of placenta previa (P < 0.05). There were 5% (12) pregnant women whose total amount of bleeding was ≥ 1000 mL in the group of fetal macrosomia, 8% (11) in the group of twin pregnancy, 11% (8) in the group of placenta previa. No hysterectomy was done among the women. The incidence of side effects in the three groups was 2.6%, 11.5% and 7.0% resp. Vomiting was frequently seen in the latter two groups, but recovered soon without treatment. Conclusion: Hemabat can significantly reduce the amount of bleeding during the cesarean section in pregnant women with high hemorrhagic risk factors and can be used with oxytocin as firstline medicine to prevent hemorrhage during and after delivery.

This paper aims to explore the clin. anal. of carbetocin combined with romethamine for preventing high-risk postpartum hemorrhage after caesarean section. 76 Cases of high-risk factor caesarean section puerpera were divided into control group and treatment group(n=38). In control group, the puerpera were injected with oxytocin 20 IU right after delivery, and i.v. infused with carbetocin 100 μg. Based on the treatment of control group, the treatment group was injected with romethamine 250 μg. The hemorrhage amount, side effect of drug and exhaust time in both groups were observed during operation, 2 h and 24 h after operation. The hemorrhage amounts during operation, 2 h and 24 h after operation in treatment group were significantly less than those of control group(all P<0.05). The exhaust time of treatment group was shorten than that of control group(P<0.05). There were no significant differences in drug side effects between two groups. Carbetocin combined with romethamine has definite effect and less side effect for preventing high-risk postpartum hemorrhage after caesarean section.

OBJECTIVE: The paper explored the clin. efficacy of intraoperative injection of carboprost tromethamine to prevent postpartum hemorrhage in cesarean sections with placenta previa. Methods: The paper selected 54 cases of cesarean delivery with placenta previa who were treated in the hospital from March 2013 to March 2015 as the research subjects, and were randomly divided into observation group and control group, 27 cases in each group. The mothers in the observation group were injected with carboprost tromethamine in the uterine body after the newborn was delivered, while the women in the control group were given oral misoprostol. The paper observed and recorded the occurrence of bleeding and adverse reactions in the two groups during and after operation, within 2 h and within 24 h. Results: The intraoperative blood loss of patients in the postpartum observation group, and within 2 h and 24 h after the operation were lower than those in the control group, and the difference was statistically significant (P < 0.05); the incidence of maternal adverse reactions was significantly lower in the observation group than in the control group. The difference was statistically significant (P < 0.05). Conclusion: Intraoperative injection of carboprost tromethamine during cesarean delivery with placenta previa could effectively prevent postpartum hemorrhage, reduce the amount of intraoperative and postoperative bleeding, and improve the quality of life of the woman.

Methods A total of 130 women after full-term delivery were chosen from the hospital from Oct. 2014 to Oct. 2016, then they were divided into control group and observation group according to random number table, 65 women in each group. The women in observation group were treated by oxytocin combined with carboprost tromethamine. The amounts of postoperative hemorrhage at 2 and 24 h after operation, the time of gastrointestinal function recovery, the first average urination time, and the incidence rates of adverse effects in the two groups were compared. Results The amounts of postoperative hemorrhage at 2 and 24 h after operation in observation group were statistically significantly less than those in control group (t = 3.14, 3.30, P<0.05). The time of gastrointestinal function recovery in observation group was statistically significantly shorter than that in control group (t = 2.74, P<0.05). The first average urination time in observation group was statistically significantly better than that in control group (F = 3.78, P<0.05). There was no statistically significant difference in the incidence rates of adverse effects between the two groups (χ2 = 0.97, P>0.05). Conclusion Oxytocin combined with carboprost tromethamine used for full-term delivery after cesarean section can effectively reduce the amount of postoperative hemorrhage, promote recovery of gastrointestinal function and urination as soon as possible, and it doesn′t increase the incidence rates of adverse effects, the value is superior to oxytocin.

To explore the clin. efficacy and safety of carboprost tromethamine combined with Barkri balloon in the treatment of postpartum hemorrhage. A total of 90 patients with postpartum hemorrhage admitted to our hospital from Jan. 2015 to Dec. 2016 were selected and randomly divided into a control group and an observation group with 45 cases each. In the control group, a Bakri balloon was injected from the vagina or uterine incision during cesarean section, and 300mL of normal saline was injected. Patients in the observation group were injected with carboprost tromethamine for combination therapy on the basis of the control group. Both groups of patients were treated for 24 h. The clin. efficacy of the two groups of patients was compared, systolic blood pressure (SBP), diastolic blood pressure (DBP) and blood oxygen saturation (SaO2), blood loss and adverse reactions were compared for safety evaluation of the combination. Results: After treatment, the total effective rates of the observation group and the control group were 93.33% and 80.00%, resp., and there was a statistical difference between the two groups (P < 0.05). There was no statistical difference in the amount of bleeding before treatment between the two groups of patients with postpartum hemorrhage (P > 0.05). After 2h and 24h of hemostatic treatment, the bleeding volume of the observation group was significantly less than that of the control group (P < 0.05). There was no significant difference in SBP, DBP and SaO2 between the two groups of patients with postpartum hemorrhage before and after treatment (P > 0.05). There was no significant difference in the occurrence of adverse reactions between the two groups (P > 0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups. Carprost tromethamine combined with Bakri balloon has good clin. efficacy and high safety in the treatment of postpartum hemorrhage, and it is worthy of clin. application.

Totally 90 cesarean section women were involved and randomly divided into normal saline (N) group, ondansetron (O) group and midazolam (M) group. SBP, DBP, HR and SpO2 were observed before, 0 and 5 min after carboprost tromethamine injection (T0, T1 and T2, resp.). SBP and DBP were significantly higher, HR was faster and SpO2 was lower at T2 compared with those at T1 in 3 groups (P<0.05). SBP and DBP was lower, HR was slower and SpO2 was higher in M group than those in N group (P<0.05). Midazolam prevents adverse reactions in cesarean section women induced by carboprost tromethamine, and the effect is better than ondansetron.

Objective To explore the clin. effect of hemabate in prevention and treatment of postpartum hemorrhage and adverse events among high risk pregnant women receiving cesarean section, accumulate experience for clin. optimization strategy among the population. Methods From June 2010 to Oct. 2013, 78 pregnant women underwent cesarean section in Department of Obstetrics and Gynecol. in the hospital were selected, then they were divided into study group and control group according to random number table, 39 cases in each group. The cases in control group were treated by oxytocin combined with carboprost methylate suppositories, while the cases in study group were treated by hemabate. The amounts of blood loss and urine volumes during operation and at 2 and 24 h after operation in the two groups were recorded, the incidence rates of adverse events in the two groups were compared. Results The amount of blood loss during operation and at 2 and 24 h after operation in study group were statistically significantly lower than those in control group (P<0.05). Urine volumes during operation and at 2 and 24 h after operation in study group were statistically significantly higher than those in control group (P<0.05). The incidence rate of adverse reactions in study group was statistically significantly lower than that in control group (P<0.05). Conclusion For the high-risk pregnant women receiving cesarean section, the clin. efficacy and safety of hemabate in prevention and treatment of postpartum hemorrhage are better than those of oxytocin combined with carboprost methylate suppositories, which is worthy of further promotion and application in clin. practice.
