Hydroxychloroquine Sulfate Related Hypoglycemia In A Non-Diabetic COVID-19 Patient: A Case Report and Literature Review

Objective: Hypoglycemia is a serious adverse effect of hydroxychloroquine (HCQ) which is very rare in non-diabetic patients. This case report describes a non-diabetic patient without any other chronic diseases, who experienced mild hypoglycemia related to HCQ used for COVID-19 treatment. Methods: All etiologies causing hypoglycemia were investigated and a 72-hour fast test was performed. Results: A 34-year-old male patient was admitted to our hospital with a high fever, cough, and chest pain. The result of his COVID-19 PCR test was positive. He received HCQ for 10 days for the treatment of COVID-19 infection. He experienced fatigue, dizziness, severe headache, weakness and feeling of hunger after discontinuation of HCQ during his isolation at home. Before COVID-19 infection, he never experienced hypoglycemia symptoms. He did not have a history of chronic diseases, drug use, alcohol consumption, or smoking. A 72-hour fasting test was performed. He complained about headache and weakness during the 72-hour test period. The PG level was determined as 49 mg/dl during these symptoms. Concurrent insulin and C-peptide levels were <2 mU/mL and 0.553 ng/mL, respectively. ACTH, cortisol, growth hormones, liver and kidney function tests were normal. HbA1c level was 4.7% (28 mmol/mol) (Normal Range %4,5-5,7). Conclusion: Hypoglycemia may be observed as an adverse effect of HCQ used for COVID-19 infection even in patients without chronic diseases and comorbidities. We must be careful while using HCQ for these patients and must warn them about this effect. The warning about hypoglycemia effect of HCQ must be added to COVID-19 treatment guidelines. One-step complexed preparation of nitrogen and Cu co-doped oxidative active carbon catalysts Cu-N/OAC for furfural selective hydrogenation with high yield

A facile procedure for preparing copper and nitrogen co-doped active carbon (Cu-N/OAC) by one-step complexed was reported and applied in liquid-phase hydrogenation of furfural (FAL). The facile procedure resulted in high Cu nanoparticles dispersion on OAC with Cu-0 and Cu+ sites and apparently promoted the catalytic activities during furfural hydrogenation reaction. The obtained Cu-N/OAC-800 shows 99.5% FAL conversion with 98.4% selectivity to furfuryl alcohols (FOL) under reaction condition of 150 degrees C, 2 MPa and 6 h. These results indicated that the excellent catalytic performance of the catalyst was due to the synergic effects of nitrogen doping and Cu metal active sites.

Novel microencapsulated yeast for the primary fermentation of green beer: kinetic behavior, volatiles and sensory profile

The development of innovative and more cost-effective approaches of making beer throughout continuous fermentation process remains a challenging problem, which is worthy of serious exploration. The current work focuses on the application of a commercial brewing yeast (S. cerevisiae Nottingham Ale), entrapped into chitosancalcium alginate double layer microcapsules, for the production of a Pale Ale beer. During the primary alcoholic fermentation, the consumption rate of fermentable brewing sugars and dissolved O-2, estimated by the Gompertz equation, was halved in the beer obtained by encapsulated yeast in comparison with the free cell. The physical-chemical parameters of beer (i.e. pH, alcohol content, color and bitterness) were not remarkably affected by the different yeast-inoculating form. However, the volatile profiles identified by means of HS-SPME-GC-MS analysis, significantly differed in terms of terpenes, esters and alcohols content, thus proving that the yeast inoculating form may typify the odor and flavor descriptors of the green beer.

Sedentary behaviour in cardiovascular disease patients: Risk group identification and the impact of cardiac rehabilitation

Background: Sedentary behaviour (SB) is potentially an important target to improve cardiovascular health. This study 1) compared SB between cardiovascular disease (CVD) patients and age-matched controls, 2) identified characteristics associated with high SB levels, and 3) determined the impact of contemporary cardiac rehabilitation (CR) on SB. Methods: For objective 1, we recruited 131 CVD patients and 117 controls. All participants were asked about their general characteristics and medical history. SB was assessed by an objective accelerometer (activPAL3 micro). For objective 2, 2584 CVD patients were asked to fill in a questionnaire about their general characteristics, lifestyle, medical history and their SB. For objective 3, 131 CVD patients were followed over time and measured, pre-, directly post- and 2 months post-CR. Results: Objective 1. CVD patients spent 10.4 h/day (Q25 9.5; Q75 11.2) sedentary which was higher compared to healthy controls (9.4 h/day [Q25 8.4; Q75 10.29]). Objective 2. CVD patients being male, single or divorced, employed, physically inactive, reporting high alcohol consumption, living in an urban environment, having comorbidities and cardiac anxiety demonstrated a greater odds for large amounts of SB. Objective 3. The CR program significantly reduced sedentary time (-0.4 h/day [95%CI-0.7; -0.1]), which remained lower at 2-months post-CR (-0.3 h/day 195%CI -0.6; -0.03]). Conclusions: CVD patients had greater amounts of objectively measured sedentary time compared to healthy controls. Sedentarism was associated with personal- and lifestyle characteristics, and comorbidities. Participation in a contemporary CR program slightly reduced sedentary time, but tailored interventions are needed to target SB in CVD patients. (C) 2020 The Authors. Published by Elsevier B.V.

Kinetics models of transesterification reaction for biodiesel production: A theoretical analysis

The kinetics of the transesterification reaction has been explored theoretically. A general rate equation (GRE) was obtained for kinetics modeling of transesterification at the entire time of reaction based on a three-step mechanism. It has been shown that the GRE model converts to a Second-Order (SO) model at short initial times of reaction and it converts to Pseudo-First-Order (PFO) model at short initial times of reaction and high concentration of alcohol. Also, a modified form of the Second-Order model (MSO) was derived when the reaction is spontaneous (at Delta(r) G << 0 conditions or at short initial times of reaction). The accuracy of theoretical models and our assumptions was evaluated by three sets of experimental data selected at literature. It has been shown that the accuracy of PFO, SO, and MSO models followed the order of MSO > SO > PFO at short initial times while the GRE model is a suitable kinetics model at the entire times of reaction. Also, our research, contrary to what has been reported so far in published papers, shows that pseudo-first-order and second-order models are only accurate for kinetics modeling of the transesterification reaction at short initial times of reaction. Radiological and physical properties of tissue equivalent mammography phantom: Characterization and analysis methods

This article provides methods to fabricate and characterize tissue equivalent properties of mammography phantom. The characterized properties are elemental composition, effective atomic number, electron density, attenuation coefficient and CT number. Polyvinyl alcohol (PVAL), ethanol solution and graphite powder were used to develop samples of breast phantoms. First, the microstructure and elemental composition of the phantom materials were acquired from the field emission scanning electron microscopy (FESEM). FESEM micrography was a good analytical technique to indicate void spaces within the phantom. Next, the effective atomic number and electron density were calculated from the percentage of elemental composition based on the energy dispersive X-ray analysis (EDXA). Mayneord’s formula and the Phy-X/ZeXTRa software were used to evaluate the effective atomic number. The calculations show that the effective atomic number ranged from 7.137 to 7.264 and 6.75-7.05 for Mayneord and Phy-X/ZeXTRa approach respectively. The electron density ranged from 3.191-3.209 x 10(23)/g. These values were found to be in good agreement with water and breast tissue by ICRP and AAPM standards. Then, the mass attenuation coefficients of the PVAL were calculated based on the transmission of low photons by using X-ray fluorescent (XRF) technique at 16.61-25.26 keV. The mass attenuation coefficient values were found to be close to water and breast tissue when results were compared to XCOM values. Finally, CT images were acquired to measure CT numbers, which were found to be near to CT number of breast tissues, -21.40 to 24.13 HU. Based on the ICRP, AAPM, BIRADS and XCOM standards, the study successfully showed methods to fabricate and characterized mammography phantoms.

Soaking induced discrepancies in oenological properties, flavor profiles, microbial community and sensory characteristic of Huangjiu (Chinese rice wine)

Soaking is a crucial process in adjusting initial fermentation acidity and achieving desired flavor quality of rice wines. Effects of soaking time on physicochemical characteristic of rice, flavor profiles, microbial community and sensory characteristic of Huangjiu were investigated. Soaking caused significantly changes in amylopectin content, and rice with high amylopectin content was conducive to produce esters and aldehydes during Hungjiu brewing. High-throughput sequencing showed that bacteria types were more abundant than fungi during brewing, suggesting that bacterial community might be primarily responsible for Huangjiu flavor. Differential analysis revealed that Weissella and Enterobacter contributed greatly to the accumulation of alcohols, esters and aldehydes during Huangjiu brewing, while Clostridium, Acinetobacter, Cyanobacteria, Zymomonas and Exiguo-bacterium were responsible for the rancidity. Based on spearman’s correlation analysis, Lactococcus, Weissella, Pediococcus, Lactobacillus and Staphylococcus were the key microbial genera responsible for the flavor characteristic of Huangjiu. While favorable soaking caused an increased intensity in alcohol-aroma, full body and continuation of resultant Huangjiu, insufficient soaking led to a higher intensity of bitter taste. Summarily, soaking acidity ranged from 2 to 3 g/L was beneficial for preventing rancidity and improving the production of flavor compounds in Huangjiu.

Baked red pepper (Capsicum annuum L.) powder flavor analysis and evaluation under different exogenous Maillard reaction treatment

To improve pepper powder product flavor, headspace gas chromatography-ion mobility spectrometry (HS-GC-IMS) was used to detect flavor substances in pepper powder, including dried pepper powder, baked pepper powder (BPP) and Maillard treated pepper powder (MTPP). Fifty-three flavor compounds were identified, mainly comprising alcohols, esters, aldehydes, ketones, etc. Volatile organic compound type and content in BPP and MTPPs differed from dried pepper powder, with more types present in MTPPs than BPP. Partial least-squares discrimination analysis and principal component analysis were employed to identify relationships between flavor substances and sample type, and evaluate different exogenous Maillard’s flavoring reaction affects. Dried pepper powder was positively related with most flavor compounds, and Maillard reaction substrate combinations of lysine-xylose, lysine-lactose, valine-fructose, and valine-xylose exhibited high correlation with several characteristic compounds, whereas no characteristic compounds exhibited high correlations in BPP. Thus, adding exogenous Maillard reaction substrate offers a mechanism to improve BPP flavor quality.

Improving heterogeneously catalyzed transesterification reaction for biodiesel production using ultrasound energy and petro-diesel as cosolvent

Heterogeneously catalyzed transesterification process has by far been one of the most popular methods of biodiesel production particularly for the oil feedstock containing large amount of free fatty acid. However, the major constraint of the method is the poor yield of biodiesel and a long reaction time that increased its production cost. With an objective to increase the product yield in lesser time, the current work investigated the effect of addition of ultrasound and addition of cosolvent in the heterogeneously catalyzed transesterification reaction. Three commonly used solid catalyst – calcium oxide (CaO), calcined sodium silicate (CSS) and sulfated zirconia (SZ) were chosen for study. The results suggested that there was a remarkable increase in the yield of biodiesel in comparison to the conventional mechanically stirred process. CSS showed the best catalytic activity both in first use and in repeated uses. It gave a maximum yield of 97% biodiesel in 120 minutes with 9:1 molar ratio of alcohol to oil 3 wt.% of catalyst and 10 wt.% of catalyst. For CaO and CSS a noticeable difference in biodiesel yield was observed upon addition of cosolvent. CaO gave 76% yield with cosolvent and 65% without it. SZ gave 73% biodiesel with cosolvent and 61% without. Since petrodiesel was used as the cosolvent it was not required to remove the cosolvent from biodiesel during the purification process.

Predictors of substance use in a clinical sample of youth seeking treatment for Trauma-related mental health problems

Child maltreatment and traumatic events are well established risk factors for adolescent substance use problems, but little is known about the unique contributions of etiological factors on trauma-exposed youths’ pre-treatment substance use in clinical settings. This study examined associations between substance use and risk and protective factors measured across multiple ecological levels among a unique sample of youth seeking treatment for trauma-related mental health problems in child advocacy centers. Participants were adolescents (N = 135; 85% female; 60% white, 31% black) aged 13-17 years (M = 15.4) with >= 1 experience of child maltreatment or other interpersonal violence, current substance use, and >= 5 PTSD symptoms. Youth and caregivers completed validated questionnaires and clinical interviews at a pre-treatment assessment in a randomized controlled trial of a treatment for co-occurring traumatic stress and substance use. Negative binomial regression models identified different patterns of risk and protective factors for alcohol and cannabis. Clinical implications of these results are discussed, including the potential targets for integrated psychotherapies that address co-occurring substance use and traumatic stress in youth.
