One-year Follow-Up Results with Hydrogel Implant in Therapy of Hallux Rigidus: Case Series with 44 Patients

Background The Cartiva implant (CI) is being increasingly used in the surgical therapy of hallux rigidus. Despite a growing number of studies, numerous questions regarding patient selection remain unanswered. Patients and Methods As part of a retrospective case series with prospective follow-up (average follow-up period: 12 months), a total of 44 patients (male/female = 16/28; mean age at the time of surgery: 55.4 years) with 44 CI were analysed (VAS, EFAS-, AOFAS-score). Using a correlation analysis and a machine learning algorithm, risk factors for therapy failure were investigated. Results The overall survival rate of the CI was 93% at 12 months. The VAS, EFAS and AOFAS scores showed a significant improvement in comparison to the preoperative condition. The mobility of the metatarsophalangeal joint showed no increase. Patients with a medium osteoarthritis grade and a medium level of clinical restraint showed the greatest improvement in relation to their preoperative condition. Conclusion The CI can be seen as an effective therapy for hallux rigidus. Nonetheless, realistic patient expectations must be communicated.

Early life stress induces hyperactivity but not increased anxiety-like behavior or ethanol drinking in outbred heterogeneous stock rats

Early life stress is known to impact vulnerability to psychopathological disorders in adulthood, including anxiety and alcohol use disorder (AUD), but the mechanisms underlying susceptibility to these outcomes are not fully understood. In the current study, we used adolescent social isolation (ASI) to determine whether Heterogeneous Stock (HS) rats, an outbred model used for genetic fine-mapping, could be used to study the genetics contributing to ASI-induced anxiety- and AUD-like behavior. We isolated (ASI) or group-housed (adolescent group-housed; AGH) 64 male HS rats at 4 weeks of age. After 5 weeks in these housing conditions, multiple anxiety and coping/despair-like behaviors were measured. All rats were then individually housed and assessed for voluntary ethanol self-administration. At euthanasia, synaptoneurosomes were isolated from a subset of brains to examine the expression of two proteins associated with alcohol drinking-related behaviors, GluA1 and SK2, in the dorsal (dHC) and ventral hippocampus (vHC). We found that ASI increased hyperactivity in the open field test relative to AGH, with no changes in other anxiety-like behaviors. Surprisingly, ASI rats demonstrated decreased immobility and increased climbing in the forced swim test relative to AGH. In contrast to prior studies by us and others, we found no difference in self-administration of 20% ethanol, with decreased ethanol self-administration in ASI relative to AGH rats at higher ethanol concentrations. Furthermore, while ASI in Long-Evans rats resulted in decreased SK2 expression in vHC synaptosomes, no differences were seen in vHC synaptosomes for SK2 or GluA1 in HS rats. These results demonstrate that HS rats are protected against many of the negative effects previously seen in response to ASI, namely anxiety-like behavior and increased ethanol self-administration. Improving the performance of a photonic PCR system using TiO2 nanoparticles

Nucleic acid amplification using polymerase chain reaction (PCR) method has been widely used in different fields such as agricultural science, medicine, pathogen identification, and forensics to name a few. Today, it seems inevitable to have a robust, simple PCR system for diagnostics at the point-of-care (POC) level. Many photonic PCR systems have been proposed in the literature that benefit from plasmonic photothermal heating to achieve the common PCR thermal cycling. However, non-homogeneous temperature distribution is a challenge in some of them. In the present work, to achieve more efficient gene amplification, the effect of adding TiO2 nanoparticles has been investigated in a photonic PCR benefiting from plasmonic photothermal heating. The system enjoys higher heating and cooling rates than those of conventional PCR systems while having much lower energy consumption. The average heating and cooling rates are 4.44 degrees C/s and 2.65 degrees C/s, respectively. The system demonstrates acceptable temperature stability and accuracy with negligible deviation from the set points. Furthermore, the system is capable of amplifying eight gene samples simultaneously which makes it a viable choice for POC diagnostics. A part of the Alcohol Oxidase gene has been amplified using the plasmonic thermal cycler. It is demonstrated that adding TiO2 nanoparticles with a proper concentration to the sample provides a more specific band that can be ascribed to more homogeneous temperature distribution owing to enhanced thermal conductivity. Dihydromyricetin improves mitochondrial outcomes in the liver of alcohol-fed mice via the AMPK/Sirt-1/PGC-1 alpha signaling axis

Alcoholic liver disease (ALD), due to the multifactorial damage associated with alcohol (ethanol) consumption and metabolism, is one of the most prevalent liver diseases in the United States. The liver is the primary site of ethanol metabolism and is subsequently injured due to the production of reactive oxygen species (ROS), acetaldehyde, and metabolic stress. Building evidence suggests that dihydromyricetin (DHM), a bioactive flavonoid isolated from Hovenia dulcis, provides hepatoprotection by enhancing ethanol metabolism in the liver by maintaining hepatocellular bioenergetics, reductions of oxidative stress, and activating lipid oxidation pathways. The present study investigates the utility of DHM on hepatic mitochondrial biogenesis via activation of the AMP-activated protein kinase (AMPK)/Sirtuin (Sirt)-1/PPARG coactivator 1 (PGC)-1 alpha signaling pathway. We utilized a forced drinking ad libitum study that chronically fed 30% ethanol to male C57BL/61 mice over 8 weeks and induced ALD pathology. We found that chronic ethanol feeding resulted in the suppression of AMPK activation and cytoplasmic Sirt-1 and mitochondrial Sirt-3 expression, effects that were reversed with daily DHM administration (5 mg/kg; intraperitoneally [i.p.]). Chronic ethanol feeding also resulted in hepatic hyperacetylation of PGC-1 alpha, which was improved with DHM administration and its mediated increase of Sirt-1 activity. Furthermore, ethanol-fed mice were found to have increased expression of mitochondrial transcription factor A (TFAM), reduced mitochondrial content as assessed by mitochondrial DNA to nuclear DNA ratios, and significantly lower levels of hepatic ATP. In contrast, DHM administration significantly increased TFAM expression, hepatic ATP concentrations, and induced mitochondrial expression of respiratory complex III and V. In total, this work demonstrates a novel mechanism of DHM that improves hepatic bioenergetics, metabolic signaling, and mitochondrial viability, thus adding to the evidence supporting the use of DHM for treatment of ALD and other metabolic disorders. Construction of graphene oxide membrane through non-covalent cross-linking by sulfonated cyclodextrin for ultra-permeable butanol dehydration

Two-dimensional (2D) lamellar graphene oxide (GO) membranes represent promising porous materials in alcohol dehydration. The separation performance can be manipulated by the physicochemical properties of interlayer channels. The relatively low flux and poor stability of GO-based membranes in aqueous environment are the major drawbacks. Herein, we intercalated a supramolecular compound, sulfobutyl-beta-cyclodextrin (SCD), into the interlayer pores/channels of GO membranes. The sulfonic acid groups on SCD molecules are able to cross-link adjacent GO nanosheets through non-covalent interactions. The GO-SCD membranes possess excellent water affinity due to the presence of high hydrophilic sulfonic acid groups. For the n-butanol dehydration, the GO-SCD/PTFE membranes with well-defined interlayer pores exhibited ultrahigh permeation flux of 12955 g m(-2) and separation factor of 1299 under 80 degrees C. The membranes remained stable in 144 h separation performance evaluation test and exhibited obviously enhanced thermal stability.

Effects of algae subtype and extraction condition on extracted fucoxanthin antioxidant property: A 20-year meta-analysis

Algal fucoxanthin as a carotenoid pigment possesses various health benefits, among which, the antioxidant property is one of the most explored. Current research indicated that algal fucoxanthin is generally extracted from different subtypes (microand macro-algae) under varying extraction conditions. However, it lacks information whether algae subtypes and extraction conditions present a remarkable impact on the antioxidant property of the extracted fucoxanthin. In this study, the effects of algae subtype and extraction condition (i.e., extraction solvents, temperature, time, pressure and illumination condition) on fucoxanthin antioxidant property were investigated by performing a meta-analysis. The subtotal standard mean difference (SMD) of the microalgae and macroalgae subtypes were 12.59 (95% confidence interval (CI): 3.63-21.56) and 7.20 (3.44-10.96), respectively, presenting an overlapping range. This suggested that no statistically significant differences existed in the fucoxanthin antioxidant property extracted from two subtypes, which was consistent with the results from the subgroup analysis and meta-regression. Similar observations were found for algal fucoxanthin extracted by alcohols (SMD (CI):7.18 (3.36-11.00)) or alkanes (SMD (CI):11.88 (3.62-20.15)). Moreover, the employed extraction conditions including extraction time (SMD (CI) for >= 60 min vs <60 min: 8.03 (3.55-12.50) and 7.97 (2.49-13.45)), pressure (SMD (CI) for normal vs pressurized: 7.68 (4.00-11.35) and 10.64 (0.21-21.03)), and illumination (SMD (CI) for dark vs normal: 6.91 (2.31-11.50) and 9.45 (4.17-14.73)) showed no statistical influence on fucoxanthin antioxidant property. However, extraction at higher temperature produced stronger fucoxanthin antioxidant property (SMD (CI) for room temperature vs >= 40 degrees C: 31.43 (12.27-50.59) and 7.21 (3.69-10.74)). Additionally, the fucoxanthin antioxidant property exhibited a positive concentration-dependent correlation according to meta-regression analysis. Our findings provide suggestions for fucoxanthin extraction from algae under various conditions and give insights to its application as an antioxidant. As more data become available in the future, data analysis could be updated for more robust comparisons.

Societal development and the alcohol-attributable burden of disease

AimThis study aimed to examine if there is an interaction between alcohol use and human development in terms of their associations with alcohol-attributable health harms. Interactions between HDI and adult per-capita consumption (APC) affecting alcohol-attributable harms were assessed using likelihood ratio tests. Differences in alcohol-attributable harms per litre of APC between HDI groups were assessed using regression analyses and a reference group of low HDI. FindingsAPC is associated with alcohol-attributable deaths, YLL, YLDs and DALYs lost, while HDI is associated with alcohol-attributable deaths, YLL and DALYs lost. Statistical analyses indicated there is an interaction between HDI and APC in their associations with alcohol-attributable deaths, YLL and DALYs lost per 100000 people. The alcohol-attributable burden was highest in low HDI countries, with 11.65 [95% confidence interval (CI) = 10.75, 12.40] deaths and 495.61 (95% CI = 461.83, 569.23) DALYs lost per 100000 people per litre of APC, and lowest in very high HDI countries, with 4.15 (95% CI = 2.46, 5.71) deaths and 200.31 (95% CI = 122.78, 265.10) DALYs lost per 100000 people per litre of APC. However, no statistical differences between low and very high HDI groupings for these burdens were observed. ConclusionsThere appears to be an interaction between the Human Development Index and alcohol use in their associations with alcohol-attributable deaths, years of life lost and disability-adjusted life years lost but not with alcohol-attributable years lived with disability. Alcohol appears to have a stronger harmful impact per litre of alcohol consumed in lesser developed countries than in developed countries.

An efficacy trial of adaptive interventions for alcohol use disorder

Background: Adaptive interventions, sometimes referred to as stepped care, are those interventions in which the type or dosage of treatment offered to patients is tailored to baseline clinical presentation and then adjusted over time in response to patient progress or lack thereof. Currently, no adaptive brief interventions exist specifically for alcohol use disorder (AUD). Method: This study used a sequential multiple assignment randomized trial design with 160 individuals with AUD recruited both locally and nationally who had a goal to reduce or abstain from drinking. Participants received brief advice (BA) and then the study reassessed them three weeks later; the study randomized those who did not respond to BA, defined as reducing their drinking to low-risk guidelines, to two session of motivational interviewing (MI) or more BA. The study then reassessed participants at week 8. The study re-randomized non-responders to receive either MI alone or MI plus behavioral self-control therapy (BSCT), also referred to as coping skills therapy, and evaluated participants at week 13. Results: Overall, participants receiving any BSCT made the greatest reductions in drinking. Participants who received MI at week 4 and BSCT at week 8 outperformed all other groups. Conclusion: Findings reveal that prolonged treatment, more sessions, and/or a specific combination of MI and BSCT provided optimal outcomes. Future research should determine whether such an algorithm holds across heterogenous groups of individuals with AUD.

Tissue-specific transcriptome recovery on withdrawal from chronic alcohol exposure in zebrafish

Alcohol consumption can lead to a wide range of systemic disorders brought about by transcriptional changes. Recent studies have documented altered behavior and physiology in zebrafish exposed to alcohol. In this work, we have identified the changes in the zebrafish transcriptome in response to chronic alcohol exposure. We have further followed the extent of transcriptional recovery upon withdrawal from alcohol and found evidence of tissue-specific responses. Our results indicate a greater extent of recovery of the brain transcriptome compared to the liver. We identify two distinct classes of genes in response to withdrawal from alcohol exposure – those that recover their pre-alcohol expression profile versus those that retain altered expression even after the fish are removed from the alcohol environment. Finally, we have examined gender-specific responses to alcohol exposure in zebrafish and find evidence for distinct alcohol tolerance levels. Upon chronic alcohol exposure, a higher percentage of genes show perturbation in expression profile in males compared to females. Female fish also recover better with more genes regaining the control expression level upon withdrawal from alcohol. Overall, our work identifies genes and pathways perturbed by exposure to alcohol, and demonstrates the extent of gender- and tissue-specific transcriptional changes associated with chronic alcoholism and withdrawal. Preventing E-cigarette use among high-risk adolescents: A trauma-informed prevention approach

Exposure to childhood trauma increases the risk of tobacco use during adolescence. Recent studies have also reported potentially increased vulnerabilities to electronic cigarette (e-cigarette) use among youth with a history of childhood trauma. While empirical evidence supporting the relationship between childhood trauma and adolescent e-cigarette use is emerging, few effective preventive interventions are available to curb e-cigarette use among adolescent victims of childhood trauma. This article reviews current evidence with respect to how childhood trauma could increase risk for nicotine dependence and e-cigarette use in adolescent populations. Furthermore, this paper describes the development, design, and implementation of Rise Above (RA), a randomized, controlled trial of a trauma-informed, e-cigarette preventive intervention. Lessons learned are also discussed, including the challenges of implementing evidence-informed prevention work within communities vulnerable to traumatic events.
